# Filling out the CLoA

# Retailer

Example
Example

Field Description
Name Your current provider's business name
Address Your current provider's company address (you can find this on Companies House)
Email Your current provider's contact email (you can typically find this on a bill)

The New Retailer box has been pre-filled for you.

# Site address to register against numbers

Example
Example

The address that your current provider has registered on their systems as to where your number is installed.

The site, installation or emergency addresses may different from your billing or postal addresses. A common cause of porting rejections is the site address being incorrect so we always suggest that you contact your current provider to clarify what they have on file for your number.

Regardless of the type of service, all UK geographic numbers have an associated address, although the name it goes by can vary (site address, installation address, emergency address, etc.). In the case of a VoIP service, it should normally be the end user's billing address.

# Numbers to be ported

Example
Example

This section must contain all the numbers to be ported. Please enter all the numbers in full UK national format, without the leading international prefix. (e.g. 020 7946 0123).

There are 3 columns to provide you space to enter the numbers you need, please start with the first one. If you need additional space for more numbers, you can finish adding more on the second page of the CLoA.

In the case of a sequential block of numbers, the entire block must be entered, even if you only wish to use part of it. Number ranges must always be ported between providers in their entirety. It may be possible to split the block up into smaller sets, but this would need to be approved and arranged with your current provider before the port went ahead.

# Main billing number (MBN)

Example
Example

This would be the main number in use with your current provider. If you only have one number, this number is the MBN.

Otherwise if you are porting multiple numbers, you may need to confirm with your current provider which number is the MBN. The MBN tends to be the main number you advertise for your business, or the first in a range.
